I have developed the above forms over the course of several years. I find they help me keep track of individual student growth and assist me when it is time to do report cards and have parent conferences. I offer them as a tool for other teachers to use in their own classrooms. I welcome your feedback, or ways you may have modified them to meet your own needs.

Statement of Use

If you use the forms, word lists, PowerPoint shows, etc. from this site in your classroom, or school, I request that you leave my name on them as author, or at least reference them as "based on materials developed by Jill Perkins." You are welcome to link to this site, but I would request that you notify me of the link.

         
   


   

To save the files above to your computer, follow the steps below:

1. If you are using Netscape Communicator (Navigator), RIGHT click on the document you want and choose "Save link as..." and then locate a place on your hard drive to save it to.

2. If you are using Internet Explorer, RIGHT click on the document you want and choose "Save target as..." and then locate a place on your hard drive to save it to.

After this, go to the location you saved the files into and double-click on one of them. It should open it in Acrobat Reader, or Word, depending on which file you downloaded.
